Use of the Expatriate Claim Form

This claim form is used to claim expenses for medical treatment related to an injury or illness of an Expatriate, their partner and any dependants.

In order to successfully submit this claim form to Comcover you will be required to complete a number of mandatory fields and supply supporting documentation. There are a number of qualifying questions and help text throughout the claim form to assist you in completing the form.

Before you start please make sure you have:

• Details of medical expenses to be claimed.
• Payment receipts or invoices for expenses to be claimed or
• Bank statements demonstrating payment of expenses.
